Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker on hose outside of back door entrance to kitchen.
Food-contact surface soiled with food debris, mold-like substance or slime. Observed black and green mold-like substance around dispensing nozzles of soda machine at waitress station.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed no employee responsibility reporting agreement for 7 employees: 3 cooks, 3 servers, and 1 dish washer.

Single-service articles improperly stored. Observed boxes of single service to go plates, cups, lids, and gloves stored on floor in dry storage area.
