Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed SALMON SALAD (55F - Cold Holding); CHICKEN SALAD (60F - Cold Holding); HARD BOILED EGGS (61F - Cold Holding) at front counter. Manager moved items to another cooling unit for rapid chill.
Time/temperature control for safety food prepared from/mixed with ingredient(s) at ambient temperature not cooled to 41 degrees Fahrenheit within 4 hours. Observed BEANS (53F - Cooling) at reach in cooler made two days prior as per manager.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach in cooler at front counter not cooling properly, left thermometer in for five minutes with a temperature of 58F.
Equipment and utensils not properly air-dried - wet nesting. Observed plastic containers at ware washing area shelf wet nesting.

No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand washing sink near three compartment sink. Paper was provided.
Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed garlic and tomatoes soup with no date marking at reach-in cooler at preparation area. As per operator, made on Saturday (more than 24 hours ago). Coached operator on correct procedure.
Bathroom located inside establishment not completely enclosed with tight-fitting, self-closing doors. Observed at bathroom used by employees.
Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Observed at preparation and front counter area.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee wash, rinse and sanitize dishes/metal bowl strainers at triple sink, then proceeded to place on gloves to portion roasted broccoli into plastic containers. Employee changed tasks without washing hands prior to placing on gloves. Discussed with Manager regarding proper handwashing requirements and provided " When You Wear Gloves" handout to keep displayed within kitchen area.
