Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook washed hands with gloves on did not use soap then proceeded to go back to prepping, educated on proper handwashing, the cook removed her gloves and correctly washed hands.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no vacuum breaker on the hose at the mop sink.

Food stored on floor. Observed one case of Mello Yello and one case of root beer for the soda dispensers on the floor in the storage closet, operator removed. Observed a container of base on the floor in the storage area, operator removed.

Single-service articles improperly stored. Observed one case of foil sheets located under shelve on the floor, manager removed.
