Toxic substance/chemical improperly stored. Observed quaternary sanitizer tablets stored next to measuring cups on shelf above reach in freezer. Operator removed to properly store.

Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up in reach in freezer in back room area. Operator stated reach in freezer was repaired yesterday and she is in process of removing build up of ice.

Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. Operator doesn't have the correct test strips for the high temp dishmachine. Operator contacted vendor.
Hole in or other damage to wall. Hole in ceiling in prep room. Operator stated the ceiling piece will be installed soon.
