Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee cut and place cooked naan bread into basket with bare hands. Discussed with employee, making sure that they are wearing gloves or using tongs to handle the cooked naan bread
Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment.

Establishment added additional equipment, different than layout approved back in 2010. Buffet line added in dining room also server station with coffee maker and ice machine added in customer area. Closest hand sink is either at the bar or in the kitchen. Back hallway has four reach in cooler and coolers and freezers Hand sink on cook line removed and replaced with tandoor oven. Three additional prep tables added in kitchen Hand wash sink left to hood. System was removed and tandoor oven is present.

Server handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Employee was cleaning dining room tables and entered the kitchen and engaged in chopping green peppers and onions. I informed the operator and she had the employee to wash and dry his hands at a designated hand washing sink.
