Raw animal food stored over/not properly separated from ready-to-eat food. In flip top reach in cooler in kitchen, container of raw tuna stored over cooked chicken. Employee put raw tuna on bottom shelf.

Employee stated he had been in cooler two hour prior and thinks the door did not fully shut. He put all Time/Temperature control for safety foods in freezers to bring down temperature.

Test kit for measuring the concentration of the sanitizing solution not used in accordance with the manufacturer's instructions or employee unaware of proper use. Test strips for dishwasher at bar expired last month. Operator has already ordered new ones.
Carbon dioxide/helium tanks not adequately secured. In back storage room, one co2 tank not secured.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Multiple gaskets on reach in coolers/reach in freezers soiled/stained or torn.

Toxic substance/chemical improperly stored. In mop sink closet in kitchen, multiple different cleaners stored on shelf above clean wiping clothes. Operator moved all chemicals to floor. Also, in storage room between restrooms, multiple different cleaners stored on shelf above single service items like paper plates and paper towels. Operator will rearrange closet today.
Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. According to cooling calculator, pulled pork will not cool to proper temperature before the 4 hour mark. Pulled pork portions are closed tightly in uncover metal pan in reach in cooler in kitchen.
Raw fruits/vegetables not washed prior to preparation. In top of flip top reach in cooler in kitchen, avocado being used with sticker still present in skin.
Single-service articles improperly stored. In storage closet between restrooms, paper towels and case of plastic to go container lids on floor. Operator stated she will be rearranging closet today.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. In kitchen, Sanitizer Bucket (Quaternary 0ppm). Operator remade sanitizer bucket, rechecked at 150ppm.

Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Establishment has "poke bowl" on menu, which raw tuna is an ingredient. Menu does not identify this product as raw. Operator removed old menus and reprinted menus to reflect item as raw.
