Equipment in poor repair. Observed warped/damaged door gaskets on reach in coolers in kitchen during inspection
Floors not constructed to be easily cleanable. Sealant on concrete floor is peeling and is no longer easily cleanable.
Hole in or other damage to wall. Observed holes in wall back by water heater.
In-use utensil not stored on a clean portion of food preparation or cooking equipment. Observed pizza paddle stored on floor. Operator had employee clean and store correctly off floor.
No handwashing sign provided at a hand sink used by food employees. No hand wash signage at hand wash sink by back office. Operator hung new sign during inspection.

Wet mop not stored in a manner to allow the mop to dry. Observed mop held in bucket not set to easily dry.
