In-use ice scoop stored on soiled surface between uses. Observed ice scoop stored directly on top of the ice machine. Employee removed it and placed it in a clean container that's cleaned and disinfected on a daily basis.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the walk in cooler next to the walk in freezer observed observed shredded chicken (49F - Cold Holding); cooked pasta (49F - Cold Holding), as per employees food was transferred to unit less than 2 hours ago. Operator transferred all food to the walk in cooler next to it to rapidly cool.

Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Observed Dishwasher machine out of service, also observed a sign on the DW stating that the machine is out of service.
