Rodent activity present as evidenced by rodent droppings found. Observed approximately 63 rodent droppings in the following areas: 48 behind chest freezer at dry storage entrance, 10 along base of wall in dry storage area, five on floor and under inoperable double door cooler in dry storage area.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cooked chicken in walk-in cooler reading 52F for 30 minutes per operator. Operator placed chicken in freezer for rapid cooling during inspection.
Floor area(s) covered with standing water. Observed water in floor in mop sink room.
Floor not cleaned when the least amount of food is exposed. Observed buildup of debris on floors under equipment.
Floor tiles missing and/or in disrepair. Observed hole leading to outside in floor in far back room with old equipment.

Wet mop not stored in a manner to allow the mop to dry. Observed wet mop by back storage area not hung to dry properly.
