Observed employee enter kitchen from front customer counter area, begin sanitizing counters and touching single use items, no handwash. Manager spoke to employee who washed hands properly before continuing work.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Attached hood and tops of equipment on main cook line with accumulated dust.
No handwashing sign provided at a hand sink used by food employees. Nearby bathrooms shared with guests.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee don gloves to handle food after touching face. Spoke with employee and operator, reminded operator that cleared and easy to access handwash sinks are required to encourage proper handwashing. Operator restored handwash sink access, employee properly washed hands and returned to work.
Handwash sink not accessible for employee use at all times. Door propped open between front and back of kitchen is blocking front of kitchen handwash sink. During this time, observed employee fail to wash hands when needed. Operator removed prop from door, allowing access to sink for employees.
Cutting board has cut marks and is no longer cleanable on front prep line.
Food stored in a location that is exposed to splash/dust. Boxed of bananas stored under handwash sink/triple sink in back prep area. Operator removed.
