Handwash sink not accessible for employee use at all times. Handwash sink blocked by pans on top of and in front of sink. Inspector had manager unblock the sink during this inspection.
Bowl or other container with no handle used to dispense food. Single-use condiment cup without handle stored down inside pan of cheese in top of refrigerated make-table. Manager discarded the single-use cup during this inspection.
Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. License number not displayed in 2 inch numbers on side of unit.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on reach-in freezer door soiled with mold-like substance.

Single-service articles improperly stored. Two boxes containing single-use trays stored directly on floor in storage truck.
Time/temperature control for safety food thawed in an improper manner. Ziplock bag containing frozen chicken thawing at room temperature in pan near cookline. Manager moved the pan of chicken to a cooler to continue to thaw.

Toxic substance/chemical improperly stored. Observed degreaser and Clorox wipes next to crisco oil and a can of black beans, operator moved to different area.

Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed feta dip, pizza sauce and falafel were made with no date marks.
Test kit for measuring the concentration of the sanitizing solution not used in accordance with the manufacturer's instructions or employee unaware of proper use Observed operator using an expired testing strips from 7/2023, used my testing strips to test sanitizer.

Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed no glass or screen in windows of unit. Awning only. Unit is not protected from flies. Observed 4 flies flying in and out of unit landing on food contact surfaces during inspection. Operator open and operating with door open and windows open no fly protection available. Operator states he has screens available at his house. Follow day callback per Kimberly Torbert at 2:02 PM.
Raw animal food stored over canned/bottled drinks. Observed container of raw beef over bottled drinks and condiments. Operator rearranged beef to bottom shelf.
Handwash sink used for purposes other than handwashing. Observed clean dishes stored in handsink. Operator removed.
Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. Observed operator using bleach as sanitizer and only has quaternary sanitizer test strips.
No paper towels or mechanical hand drying device provided at handwash sink. Operator added.
