Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Bacon in reach in cooler at 45F( Cold Holding ). Per manager bacon has been taken in and out of cooler since 12PM. manager placed bacon in freezer.
Food stored on floor. Mustard bottle observed on floor under counter. Manager picked up bottle and placed it on the counter Water found on floor under counter manager picked up water and placed it on soda boxes under counter. Box of bread in packaging found on floor under counter manager picked up the box and placed it on the counter.
Outer openings not protected during operation and vermin and/or environmental cross contamination present. Door were open observed 3 flying insects inside. Manager closed door
