Single-use gloves not changed as needed after changing tasks or when damaged or soiled. 1. Employee left kitchen through rear exterior door with gloves on then returned to cookline and prepared a customers order without changing gloves. Reviewed glove usage with operator.

Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Raw chicken stored over raw ground beef inside Frigidaire reach in freezer in outside storage room. Operator moved raw chicken to bottom shelf and stored correctly.
Handwash sink not accessible for employee use at all times. Towels and strainer inside hand wash sink in kitchen. Operator removed
