At walk in cooler - cut lettuce 48F- cold holding, cut mixed greens 48F- cold holding, cooked eggs 47-48F- cold holding, milk 47-48F- cold holding- food not prepared or portioned today- food out of temperature for 1 hour as per operator- operator started to place food in walk in freezer to quick chill.

Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Cook removed outer gloves with their hands after handling raw burgers then removed inner gloves put on new gloves without washing hands - operator instructed employee to wash hands and the glove changing policy.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. In ice machine in back kitchen.
