Handwash sink not accessible for employee use at all times. Handwashing sink has a tray stored on sink covering sink. Employee removed tray.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice machine is soiled n kitchen area.
In-use knife/knives stored in cracks between pieces of equipment. Knife stored between wall and food preparation table on cooks line. Chef removed knife's and placed at dishwasher.

Handwash sink not accessible for employee use at all times. Most hand sinks has containers in them, they where moved or other area. Cooks line had pan on top of sink with food items, operator moved the pan.
