Accumulation of dead or trapped insects in control devices. Upon inspection, observed approximately 40 dead flying insects in traps in the following locations: approximately 20 in trap outside of walk-in cooler door, approximately 20 in trap near back door.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Upon inspection, observed dishmachine chlorine sanitizer reading 0 ppm.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Upon inspection, observed cooked chicken 46f, sliced turkey 46f, shredded cheddar 46f, salsa 46f in lower make bar cooler, less than 4 hours per employee.

Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Upon inspection, observed pan of cooked chicken in make bar lower reach in cooler not date marked. Employee stated not over 7 days.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Upon inspection, observed mold-like substance on ice chute inside of ice machine.
Carbon dioxide/helium tanks not adequately secured. Upon inspection, observed carbon dioxide tank and nitrogen tanks not secured in rear of establishment.
Equipment and utensils not properly air-dried - wet nesting. Upon inspection, observed storage pans and lids wet nesting in dish area of establishment.
Fly sticky tape hanging over food/food preparation area/food-contact equipment. Upon inspection, observed fly sticky tape over food prep table in kitchen.
Ice buildup in reach-in freezer and/or walk-in freezer. Upon inspection, observed ice buildup in walk-in freezer.
