Nonfood-contact surface soiled with grease, floor around grease container in back kitchen area is leaking. Manager states it has been reported to upper management.
A return visit. The problems from the previous inspection had been dealt with.

Ceiling/ceiling tile shows damage or is in disrepair. Hole in ceiling tile above bag in box drink system. Ceiling tile damaged at wall near water heater. One Ceiling tile in dining room had black mold-like substance.
Cove molding at floor/wall juncture broken/missing. Following areas: adjacent to back door, beside walk in cooler, under triple sink, wall before triple sink entrance.
Floor area(s) covered with standing water. Observed standing water on floor at/near/from back door through floor in front of walk-in cooler and freezer.
Floor tiles missing and/or in disrepair. Observed broken/missing floor tile in following locations: Tile Inside mop sink, tile in front of back door and threshold, tile in front of triple sink.

Carbon dioxide/helium tanks not adequately secured. Observed carbon dioxide tank not secured in dry storage area near soda syrups.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee cell phone stored on food prep table next to charbroiler. Employee removed cell phone.
Water leaking from pipe and/or faucet/handle. Observed water leaking from pipe underneath three compartment sink.
