Observed quatanery ammonium sanitizer solution in three compartment sink testing at 500+ppm. Employee dumped sink and filled with quatanery ammonium solution testing at 200. Triple Sink (Quaternary 500+ppm); Triple Sink retest (Quaternary 200ppm)
Raw animal food stored over/not properly separated from ready-to-eat food. Observed box of raw shell eggs stored over box of ready to eat buttermilk mix. Person in charge moved all food items to proper storage levels.
Handwash sink used for purposes other than handwashing. Observed brown stains in hand wash sink by to-go window. Person in charge stated the iced tea was dumped in hand wash sink the night prior.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Observed one team member on cook line person in charge stated was only on second day. Team member did not have food handler training or employee reporting agreement signed. Person in charge had employee sign reporting agreement.
Employee drinking from an unenclosed beverage container in a food preparation/clean dishware area and the container does not have a dispensing mechanism to prevent hand contact with lip contact area of drink container. Observed employee beverage with no lid on top of paper towel dispenser above hand wash sink near to-go window. Person in charge voluntarily discarded beverage.

Employee handled soiled dishes or utensils and then picked up plated food, served food, or prepared a beverage without washing hands. Employee at drive thru window picked up soiled ice scoop off floor, then began making drinks, without washing hands between task.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On melted ice baths on prep line, per manager have been sitting there approximately 3 and a half hours. (52F - Cold Holding); shredded cheddar cheese (60F - Cold Holding). In reach in cooler next to cook line, per manager the door to the cooler was being open and closed for approximately 4hours. (49F - Cold Holding); shredded cheddar cheese (53F - Cold Holding). All items placed in walk in freezer, all re temps under 41 degrees.

Four employees present did not have food safety training certificates.
