Observed operator touching clothing and handling money transactions from customers and not washing hands when preparing food due to having no hand wash sink available on unit

As per operator, all foods which are 1 bag of cooked sausages, and 1 bag of hotdogs and 1 bag of cooked frozen beef burgers and 1 container of slice cheese and 1 container of coleslaw in non-pre portioned cups and 1 container of bulk relish and sauerkraut and several bottled drinks and canned drinks has been prepared and/or stored at home residence and not at licensed commissary today. Operator has not been at commissary for past few months and not sure if location is still operating.

Handwash sink removed from food preparation/dishwashing area. Must be reinstalled in the same location where removed. Operator has hot dog cart with no hand wash sink. Operator stated cart was in accident and hand wash sink was destroyed and has not repaired it yet
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Operator states uses chlorine to sanitize but has no chlorine test strips
