Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee pull personal cell phone out of back pocket, scroll through, put back in pocket then put on clean vinyl gloves without washing hands. Spoke to Operator about proper hand washing.

Operator states employee files maintained in HR dept and HR unavailable at time of inspection.
