Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed fan guards in walk-in cooler next to walk-in freezer with accumulation of dust.
Plumbing system in disrepair. Upon inspection, one of four handwashing sinks noted to be inoperable with faucet missing.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit: Cold holding station on the cook line: diced ham 54° F, diced tomatoes 45° F, diced sausage links 57° F. Items moved to a reach-in cooler to cool.
No soap provided at handwash sink: Hand-wash sink on cook line does not have soap. Kitchen supervisor had an employee restock.
Bowl or other container with no handle used to dispense food: 1. Hush puppy mix has a cup and a food boat laying in them to be used as scoops. Manager removed them. 2. Flour bin has a cup laying in it being used as a scoop. Manager removed it.
In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container: Bin of panko crumbs in dry storage room has the scoop handle laying on the panko. Mag are stood the handle up.

Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature: Raw marinating chicken in pans on a sheet pan over raw hamburgers in a pan on a speed rack in the walk-in cooler. Employee rearranged the two for proper food storage.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet: Mop sink has a four way splitter on it with one hose going to a chemical dispenser. The other three connections require a backflow preventer. Even though a hose is not connected to the three threaded spouts, there is a potential for one to be attached. Therefore, a backflow preventer is required.
Food manager certification expired.
