Ceiling/ceiling tile shows damage or is in disrepair. Observed a part of the ceiling being damaged by water stains in the front self service area.
Cutting board has cut marks and is no longer cleanable. Observed the cutting board on the flip top unit on the cook line with many cut marks to where it is no longer easily cleanable.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed an employee's cellphone on a prep table on the cook line. Manager had employee remove and store correctly.
Food stored on floor. Observed a box of fries stored on the floor in the freezer. Manager placed box 6 inches off of the floor.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up on the pipes and door of the walk in freezer.

No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand sink next to walk in cooler.
Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. During sanitizer test, no sanitizer detected. Operator called technician during inspection.

See stop sale- manager discarded. Buffet- cut honey dew and peeled hard boiled eggs. Manager advised on buffet for one hour and placed product on ice.
