Food stored on floor. Bottled water on the floor in coffee storage area. Person in charge picked bottles up and stored correctly.

Establishment has no approved Alternative Operating Procedure. Observed food bare hand contact a burrito and strawberries when plating food. Discussed with employee. No visible contamination observed.

Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed hose Bibb in prep area next to walk-in cooler with attached green hose missing vacuum breaker.
