Nonfood-grade bags used in direct contact with food. Observed non food grade bags used to store fish in walk in freezer. Manager took fish out of non food grade bags and placed into ziploc bags.
Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed employee talk on cell phone with gloves and then touch raw chicken, no handwashing, no glove change. Educated employee on best practices concerning hand washing.

And shrimp 51F reach in cooler near fryer, both items placed 1.5 hours ago, manger replaced both items from items in walk in cooler and placed items that were out of temp in walk in cooler.
Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed no hose Bibb in mop sink.
Handwash sink not accessible for employee use at all times. Observed trash can blocking hand wash sink. Manager moved trash can.
Cardboard used on floor as anti-slip measure not replaced every day or when heavily soiled, whichever comes first. Observed soiled cardboard on floor near fryer. Manager replaced cardboard during inspection.
