Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Verde Salsa and Roja Salsa made the day before and not date marked, owner put a label and date marked for 12/16/25.
Ceiling tile missing. Observed ceiling tile missing in back storage area above ice machine.
Drain cover(s) missing. Observed drain cover missing in mop sink.
Equipment in poor repair. Observed gasket torn in reach in cooler on the cook line.
Faucet/handle missing at plumbing fixture. Observed faucet handle for hot water is broken and missing in women's bathroom at the hand wash sink.
Food stored on floor. Observed container of rice, beans and sugar on the floor in dining area, owner moved to table.
Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup in reach in freezer at front counter.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed microwave soiled with food debris in kitchen.
No handwashing sign provided at a hand sink used by food employees. Observed no hand washing sign at the hand wash sink in back storage area, owner placed sign above sink.

Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw eggs stored over ready to eat products in their make right side reach in cooler. Operator removed the eggs.
No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observed no certificates for their cook.
Bowl or other container with no handle used to dispense food. Observed multiple bowls used as scoops for containers of rice. Operator removed the bowls.
Floor not cleaned when the least amount of food is exposed. Observed the floor under the cook line with debris buildup.

Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed employee serving foods at counter and dining areas touching unsanitized tables and equipment at front counter then return to kitchen and touch clean utensils and food storage without washing hands. Employee was directed and washed hands before continuing.
Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed employee serving foods at counter and dining areas touching unsanitized tables and equipment at front counter then return to kitchen and touch gorilla with bare hand. Operator discarded touched foods washed hands before continuing.
