Food stored on floor. Observed two large plastic gallons of condense milk on the floor in prepped area. Operator removed and placed them on shelf.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Dishwasher (Chlorine 0ppm). Operator replaced empty container and primed new Dishwasher (Chlorine 50ppm).
Raw animal food stored over/not properly separated from ready-to-eat food. Observed at cook line reach in cooler raw chicken breast stored over precooked chicken wings. Operator removed and stored correctly.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed at outdoor ice bins.
