Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed salmon portion salmon portion thawed inside commercial ROP bag at reach in cooler, as per staff for over 24 hrs. Staff voluntarily discarded the item.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed individual butter packet (61F - Cold Holding); cut honeydew melon (50F - Cold Holding) and milk canister (50F - Cold Holding) located at breakfast buffet, due to food items not fully submerged in ice to keep temperatures at +or- 41F and below, less than 4 hours. All food items were removed and placed into reach in cooler at kitchen area. Observed butter (52F - Cold Holding); shredded cheese (45F - Cold Holding) located at walk in cooler, less than 4 hours. As per Manager, walk in cooler door was left open. Manger opened door of walk in freezer located inside walk in cooler to rapidly cool walk in cooler unit.
Warewashing sanitizing solution exceeding the maximum concentration allowed. Observed three compartment sink quaternary sanitizer at 500ppm. Advised to Manager to dilute sanitizer solution with water due to solution exceeding the maximum concentration allowed.
