Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Dish machine not reaching at least 160°F. Heat strip did not turn black. Multiple attempts made. Three compartment sink set up.
Raw animal food stored over/not properly separated from ready-to-eat food. On cook line, raw burger patty stored over ready to eat chicken. Operator moved chicken to different cooler.

Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee came from back entrance door, proceed to cook line, placed gloves to start working in food preparation without washing hands first. After manager instructed, employee discarded gloves, washed hands and put in new pair of gloves.
Sanitizer solution other than chlorine, quat or iodine not at the proper minimum strength listed in the manufacturer's instructions. Do not use equipment/utensils not properly sanitized. Triple Sink (840-sink and surface). Manager made new solution, -Sink and Surface(700 )
