Soiled dry wiping cloth in use. Observed on the cutting board on the cook line.
Standing water in bottom of reach-in-cooler near the three compartment sink.

No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Manager couldn't provide the test kit when asked to test the sanitizer bucket on the line.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at the hand wash sink next to the three compartment sink. Manager had an employee replace the paper towels.
Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed two ceiling tiles with water stains in the hallway walking towards the bathrooms.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed an employee cellphone on the cutting board on the flip top unit. Manager removed cellphone and sanitized the area.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled exterior of the microwave. Observed the side of the reach in cooler closest to the three compartment sink soiled with food debris. Manager stated that it will be cleaned when time permits.
Soiled dry wiping cloth in use. Observed two soiled dry clothes on the cutting board on the flip top unit. Manager removed towels.
