Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In top part of flip top reach in cooler near pass through window with lid open, house made ranch (46F - Cold Holding); coleslaw (45F - Cold Holding). Manager stated lid was opened during rush and shut lid.
Toxic substance/chemical improperly stored. On back storage shelf across from walk-in cooler, multiple chemicals stored on shelf above paper towels and bag- in- box soda boxes.

Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. In warmer on cook line, fried potatoes 109F. Potatoes heated approximately 15 minutes prior. Potatoes voluntarily discarded. In warmer on cook line, fried chicken bites 121F. Chicken cooked approximately 15 minutes prior. Chicken voluntarily discarded.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Vacuum breaker missing at the end of the splitter for mop sink faucet where sliver hose is attached and where no hose is attached.

Establishment did not report seating change that affects the license fee, Clean Indoor Air Act, sewage system approval or other related requirements. Establishment is licensed for 36 seats, but has additional 16 outdoor seats. Inspector submitted change of record form to add additional seats to license (license fee may increase due to the change of seats). Inspector emailed Seating Change Evaluation Form to be completed by the establishment and submitted to District Office for approval.
