Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed multiple commercially processed deli meats with no date marking when food items were opened.
No handwashing sign provided at a hand sink used by food employees. Observed no handwashing sign at either hand wash sink in kitchen. Inspector provided sign.

Observed two employees person in charge stated have worked for less than 60 days with no proof they are informed of their responsibility to report symptoms of food borne illnesses.

Floor tiles missing and/or in disrepair. Tiles missing/disrepair under dough prep area in kitchen.

Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Interior door and ceiling corners of walk in cooler have dust build up. Exterior of pizza oven has grease build up. Shelf next to walk in cooler where salad dressing is kept is soiled with dust build up.
Unwashed fruits/vegetables stored with ready-to-eat food. Unwashed box of tomatoes placed over bag of ready to eat cheese in walk in cooler. Manager moved produce.
Working containers of food removed from original container not identified by common name. Container of crust seasoning on storage rack next to pizza prep station was not labeled. Employee labeled.
