Ceiling/ceiling tile shows damage or is in disrepair. Ceiling tiles has water damage due-to a leak coming from the ceiling unto the reach in cooler.

Food stored in a location that is exposed to splash/dust. Water leaking from the ceiling onto the reach in cooler work station, establishment is unable to block off the area at this time. The operator moved the product to another cooler to assemble the pizzas until the ceiling is fixed.

Open container of Monster energy drink on the reach in cooler. Pizza station the beverage was removed.

Employee used tobacco, ate, or drank then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee was out smoking entered the work area without washing his hands. I had the 'em to stop and wash his hands before engaging in food preparation.
Single-service articles improperly stored. Pizza boxes stored on the floor near the table located by the hand washing sink. The boxes was reform off the floor.
Wall soiled with accumulated grease, food debris, and/or dust. Floor tiles cracked in the dishwasher area especially by the hand washing sink. Cove molding missing by the pizza oven and by the dough mixer in the dishwasher area.
