Per operator coleslaw was made yesterday. Cooler had been opened repeatedly during lunch. Operator placed coleslaw in walk-in cooler. Re temped coleslaw approximately 1 hour later. Re temped at 43F. At bar reach-in cooler, half n half (48F - Cold Holding) Operator voluntarily discarded half n half.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At mop sink, splitter added to mop sink. Vacuum breaker located on faucet before splitter. Vacuum breaker needs to be installed at end of splitter on non chemical dispenser side.

Rice is in large pan with another pan above it and stored close to door. Operator operator placed rice in walk-in freezer to bring down to appropriate temperature.
