Carbon dioxide/helium tanks not adequately secured. Observed : the co2 tank at the bar is not secured.

Handwash sink used for purposes other than handwashing. Observed cloths in the mid kitchen hand wash sink. These were removed.

Equipment in poor repair. Observed: the gauge on the dish wash machine for rinse, never moved during a wash/rinse cycle.
Food stored on floor. Observed: bulk oil on the floor in the storage area.
Handwashing sign provided at a hand sink used for a dump sink. Observed: at the bar a hand wash sink and a dump sink side by side, both having a sign guiding employee to wash their hands.
