Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Facility is hand-mixing and was using degreaser, not quaternary solution. Operator retrieved proper quaternary solutuin and inspector demonstrated how to mix to proper ratio. Sanitizer Bucket (Quaternary 0ppm)

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dishwasher (Chlorine 0ppm);
In-use ice scoop stored on soiled surface between uses. Observed scoop stored in a holster by the Oasis machine. Holster had heavy food debris and mold-like build-up.
