Manager took milk to walk in cooler. Also observed sausage (49 F - Cold Holding); ham (50 F - Cold Holding); salmon (50 F - Cold Holding)inside the walk in cooler. As per manager delivery came and left the door open for about 1 hour.
Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed reach in cooler ambient thermometer at the server alley was at 50F. And also observed reach in cooler at the cook line.

As per manager for less than 4 hours. Employee placed all the items in ice bath immediately.

Time/temperature control for safety food thawed in an improper manner. Observed Beef pot roast thawing at room temperature of 80 F at the prep area. Employee was coached and opened the cold running water.

Observed cook cleaning his hands with wiping cloth then continued to engage with food preparation without washing hands and changing gloves prior to. Coached manager on proper hand washing techniques.

Observed in the reach in cooler beneath the microwave at cook line pot roast (46F - Cold Holding); cheese (50,F - Cold Holding); raw steak (46F - Cold Holding), as per manager for less than 3 hours, manager instructed employee to add ice in the containers to rapidly cool down food products. Employee added ice to containers on site. At front line observed butter (50 F - Cold Holding), as per manager for about 3 hours, employee added ice to the container to rapidly cool.
Floor soiled/has accumulation of debris. Observed heavy accumulation of grease and old food debris underneath the cooking and preparation equipment at cook line area.
Food not stored at least 6 inches off of the floor. In the walking freezer observed a case of fries stored on the floor.
