Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. In the walk in cooler observed raw salmon stored in reduced oxygen packaged, manager stated that fish was recently brought out to thaw. Manager removed fish from packaging on site.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employees car keys stored on small storage area near clean bowls, plates next the fryer at cook line, manager removed it on site.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed vegetables (46F - Cold holding); cheese (50F - Cold Holding) in reach in cooler by the cook line. Manager removed products and placed the in walk for rapid cooling. Observed butter (58F - Cold Holding) at front line, manager removed it and placed them on walk in cooler for rapid cooling.
Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Manager printed forms and had each employee on duty signed.
