Ceiling/ceiling tile shows damage or is in disrepair. Observed in men's room a crack in ceiling and some slight growing from possible water damage in urinal area
Cove molding at floor/wall juncture broken/missing. Observed in ladies room, broken cove molding/tile piece to left of handwashing sink.
Old labels stuck to food containers after cleaning. Observed on drying shelf in dish room, container with label from purchase still on container. Observed operator remove label and put into dish to rewash.
Toilet/urinal not flushing/functioning properly. Observed clogged toilet in men's room. Observed operator unclogging toilet.

Wall soiled with accumulated grease, food debris, and/or dust. Observed multipurpose walls in kitchen area with black mold like substance on them

No soap provided at handwash sink. Observed no hand soap at marked hand washing sink next to large floor mixer in back room. Manager on duty stated that it was ordered about two weeks ago.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed inside ice machine some mold like substance on plastic white guard.
Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling tiles above dishwasher broken and not easily cleanable.
Exterior door has a gap at the threshold that opens to the outside. Observed gaps around back exit door leading out side.
In-use tongs stored on equipment door handle between uses. Observed tongs on fryer handle doors. Observed manager on duty removing them and putting in dish.
Toilet/urinal not flushing/functioning properly. Observed in second stall, in ladies room, the toilet is clogged and very little water inside toilet.
Water leaking from pipe and/or faucet/handle. Observed hot water side of faucet at three compartment sink with water running from it in off position.
Wet mop not stored in a manner to allow the mop to dry. Observed wet mop stored in mop bucket ringer with mop head down.
