Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Sausage crumbles 44F inside the top section of the flip top reach in cooler at the pizza station. Ice was put under the product. diced chicken (45F - Cold Holding); hamburger patties (45F - Cold Holding); hot dogs (50F - Cold Holding); American cheese slices (45F - Cold Holding) inside the top section of the flip top reach in cooler across from the cook line. The containers was put on ice.
No soap provided at handwash sink. The soap was replaced.
Accumulation of black/green mold-like substance in the interior of the ice machine. Bottom of the interior panels inside the ice machine is soiled with black mold-like substance.
