No paper towels or mechanical hand drying device provided at handwash sink. At hand wash station at far end of cook line.

Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. At mop sink, splitter has backflow preventer on hose to chemical dispensers, missing on non chemical dispenser side.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine reading approximately less then 50ppm, employee replaced sanitizer bucket for machine and ran machine several times, retested at 100ppm
Toxic substance/chemical improperly stored. Squeeze bottle of soap stored next to container of liquid butter on lower shelf of prep table in prep area, manager removed soap.
