Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Fresh snapper is used for raw fish ceviche. Operator now knows to only use frozen fish with a letter of parasite destruction. Parasite letter on site for frozen seafood served raw.
In-use knife/knives stored in cracks between pieces of equipment.
In-use tongs stored on equipment door handle between uses.
The interior of the ice machine/bin has a rusting bolt holding door hinges that allows rust to run / drip inside the machine
