Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee returned from bathroom and began to handle clean equipment and wash dishes without asking hands first.
Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. brown rice (129F - Hot Holding) in rice warmer at front line, per employee rice was made and out in warmer less then 1 hour ago, operator began to order new warmer during inspection.
Single-service articles improperly stored. Boxes of to go lids and containers on floor in back storage room.

Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. avocado cream (45F - Cold Holding) in squeeze bottle over fill line in top section of reach in cooler at front counter, per employee cream was placed in cooler about 1 hour ago, cream placed in bottom of cooler.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No test strips of any kind.
