Accumulation of black/green mold-like substance in the interior of the ice machine/bin. There is a red,mold like substance on the lip of the ice chute in the ice machine. Operator cleaned and sanitized the ice chute.
Equipment in poor repair. The door gasket on the single door make table cooler on the cook line is damaged.
Single-service articles not stored inverted or protected from contamination. There are unprotected coffee filters on top of the espresso machine. Operator discarded the exposed coffee filters.

Per operator the potatoes were cooked 3 3/4 hours prior.

Bowl or other container with no handle used to dispense food. There are plastic ramekins in containers of cut tomatoes and pico de gallo. Operator discarded the ramekins. Corrected On-Site An employee scooped ice with foam to go cups. Educated the employee on proper use of handled utensils. Corrective action taken.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. In the bottle of the single door make table commercially processed reduced oxygen packaged tuna and salmon, both labeled to be removed from the packaging before thawing are still in the packaging. Operator removed both fish from the reduced oxygen packaging.
In-use ice scoop stored on soiled surface between uses. The ice scoop is stored on the grate of the soda machine. Operator washed and sanitized the ice scoop and placed it in a clean container.
