Operator stated items had been in cooler since day before. Operator voluntarily discarded products. Operator called repair.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Sanitizer reading of the dishwasher 00ppm chlorine. Manager stoped using the dishwasher, set up the three compartment sink to properly sanitize and called the technician.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No chlorine test kit for the dish machine.

Raw animal food stored over/not properly separated from ready-to-eat food. Observers raw salmon over cooked vegetables in single door reach-in cooler at the end of the cooks line. Operator moved salmon to beneath vegetables.
Self-service salad bar/buffet lacking adequate sneezeguards or other proper protection from contamination. Observed outside salad bar with out a sneeze guard over mixed greens, melon and cut lettuce.
No soap provided at handwash sink. Observed no soap at hand washing sink in kitchen near triple sink. Operator replaced soap.
Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed ice machine bin with green slime on interior of bin.
Dumpster overflowing garbage. Observed dumpster over flowing in back dumpster area.
Garbage on the ground and/or pad around dumpster. Observed excessive garbage on ground around dumpster area.
Water leaking from pipe and/or faucet/handle. Observed leaking pipe under three compartment sink in kitchen.

Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed multiple ceiling vents with dust build up on the ventilation system in the kitchen.
Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed an employee water bottle on the cook line and in the dish area. Operator removed the drinks.
Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed an employee cooking food while wearing a watch. Operator had employee remove the watch.
Open dumpster lid. Observed the dumpster lids open. Operator closed the lids.
Single-service articles not stored inverted or protected from contamination. Observed coffee filters not inverted or protected. Operator removed the filters.
Unclean building components, attachments or fixtures. Observed black build up on the fan covers of both walk in coolers.
