Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker on splitter at mop sink.
No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink in storage area. Operator provided paper towels.

Accumulation of black/green mold-like substance in the interior of the ice machine/bin. There is a mold like substance on the interior walls of the ice machine. Operator cleaned and sanitized interior walls of the ice machine.
Equipment in poor repair. In the back kitchen the door gaskets of the double door reach in freezer and the three door reach in cooler are torn.
