Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In cook line fliptop upper, cut lettuce (45F - Cold Holding). Per manager not prepared or portioned today and in unit from walk in approximately 1 hour. Container not adequately placed to maintain in cold air. Manager removed container to walk in to rechill and replaced with new container properly placed in unit.
Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Ice bucket by soda boxes not inverted. Manager inverted.

Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed raw salmon, partially thawed, stored inside packaging indicating removal upon thawing. Operator had employee remove fish from packaging.
Water leaking from pipe and/or faucet/handle. Observed faucet leaking at harvest and ground front counter hand wash sink.
