In double door stand up reach-in cooler, milk (47F - Cold Holding); sausage (46F - Cold Holding); chili (46F - Cold Holding). Operator stated reach-in cooler has been opened continuously. Operator called to have unit serviced.
Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. In back kitchen, cell phone on slicer. Operator moved cell phone.
Floor tiles missing and/or in disrepair. At front line and prep area, floor tiles broken/chipped.
Time/temperature control for safety food thawed in an improper manner. In back kitchen, deli turkey thawing in standing water. Operator placed under running water.

Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. In chest freezer in back kitchen, plastic container with raw salmon in zipper seal bags stored on top of case of fully cooked chicken. Operator moved salmon to correct storage.
