Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Milk in walk in cooler opened three days prior, according to manager not dated. Date mark added.
Handwash sink used for purposes other than handwashing. Hand wash station in kitchen, bowl in sink. Bowl was removed from sink.
No soap provided at handwash sink. At men's restroom, no soap at hand wash station.

Paper towel dispenser at handwash sink not working/unable to dispense paper towels. At men's restroom, paper towel dispenser not working.
Ceiling tile missing. One ceiling tile missing in kitchen above water heater.
Floor tiles missing and/or in disrepair. Several floor tiles in disrepair in kitchen.
Floor, walls and or ceilings soiled/has accumulation of debris. Floor and wall by fryer in kitchen has build up.
Hole in or other damage to wall. In men's restroom behind door, hole in wall.
Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood filters have build up of grease. Side of fryer and stove are soiled with grease and food debris. Portable fan in kitchen has build up of dust.
Utensils in poor condition. Plastic spatula melted on one side, with plastic peeling off. Manager decided to discard.
A return visit. The problems from the previous inspection had been dealt with.
