Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. On cook line, fish thawed in reduced oxygen package. Employee stated thawed less than three hours. Employee started to remove fish from package.

Employee failed to wash hands before putting on gloves to initiate a task working with food. Two gloved employees came from outside, walked into cook line, changed gloves without washing hands first. After manager instructed, both employees discarded gloves, washed hands and put in new pair of gloves.
Chemical test kit not used to ensure proper sanitization of equipment and utensils when using a chemical sanitizer.
No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No quaternary test strips.
In-use utensil not stored on a clean portion of food preparation or cooking equipment. -cook line- 3 knives stored on soiled shelf, next to microwave. -preparation area- two potatoes peelers stored on soiled container. Employee removed all utensils to be wash, clean and sanitize.
Interior of oven/microwave has accumulation of black substance/grease/food debris. Cook line- soiled microwave, employee cleaned and sanitized during inspection.
Reach-in cooler shelves with rust that has pitted the surface. Entrance of cook line, tow doors reach in cooler, shelves with build up of rust.
Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer Bucket (Quaternary 00ppm), employee made new quaternary sanitizer, 200 ppm.
