Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed open gallon of milk not date marked at Reach in Cooler. Operator date marked the food items.
Handwash sink used for purposes other than handwashing. Observed multiple ice cubes at wait station handwash sink. Educated the operator to strictly use the handwash sink for handwashing.
Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed thawed fish in reduced oxygen packaged at 3-compartment sink. Operator removed the food item from reduced oxygen packaging.
Equipment and utensils not properly air-dried - wet nesting. Observed multiple wet cups stack on top of each other with no airflow at wait station. Operator removed the cups from being stacked at wait station.

Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chlorine sanitizer 0ppm on test strip. Corrected to 100ppm by priming the chlorine sanitizer.
Handwash sink not accessible for employee use at all times. Observed container of water in the prep area handwash sink. Operator removed the container of water from the handwash sink.
Required employee training provided by a third-party employee training program and operator unable to provide original certificate for employees trained. Observed photocopy of foodhandler training certificate.
Accumulation of lime scale on the inside of the dishmachine. Observed lime scale buildup in the interior of dish machine.
Employee with no beard guard/restraint while engaging in food preparation. Observed employee with no beard guard at prep area.

Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee cell phone on top of counter of Reach in Freezer across the stove. Operator removed the cell phone to a different and safe location.
In-use tongs stored on equipment door handle between uses. Observed multiple tongs at oven handle. Operator placed the tongs above the stove at shelf.
